If you live here, or are moving into this area, you are welcome to apply to register with our practice.

If you do not have a medical card you will be asked to complete a registration form giving your name, address, date of birth, and details of your previous address and doctor. This allows the Health Authority to arrange for your medical record to be transferred to us.

If you do have a medical card, please bring it with you to the surgery when you want to register as this shows all the information required.

Proof your new address will be required. This can be in the form of a utility bill, bank statement or driving licence etc.

When you register with the practice you can request an appointment with the practice nurse for a new patient health check. This allows any significant health problems or family history to be recorded prior to the receipt of your medical record and allows you to discuss any issues or concerns you may have.

You will also be given a practice leaflet which gives you all the information about the two separate GP Practices, their services provided, and the surgery opening times.
